Vulnerable Sector Screening (VSS) and Criminal Offence Declaration Information

 

As outlined in the OHF Screening Policy

All Coaching Staff, Trainers, Managers, Board Members, Association Staff and those who help on the ice are required under OMHA, OHF, and Hockey Canada rules, to be screened with a Vulnerable Sector Screening Check (VSS). 

Season 1 - VSS

Season 2 - Signed Declaration

Season 3 - Signed Declaration

Season 4 - New VSS must be acquired

Season 5 - etc....

**Your VSS or Declaration MUST be received before you can be rostered (assigned) to a team**

OBTAINING A NEW VSS:
COST:  FREE
- As a volunteer there is no cost to you, or GMHI, to obtain your VSS but it is your responsibility to have it done and submitted to our Secretary.
REQUIRED STEPS:
1. Printout and complete the GMHI_VSS_Request_Form_.pdf

2. Take letter, ID & go to the OPP (the Goderich OPP Detachment located on Highway 21 or on The Square).

The Administrative Clerk will go through the process with you, check your ID and let you know approximately how long it will take until your VSS should be complete. They will contact you when it is ready and you will need to go to pick it up.


3. Pick up & submit to the Secretary


 
PLEASE NOTE: that if your name and birth date are similar to someone who has a conviction, you may be asked to submit finger prints before they can provide the VSS. The Goderich OPP Detachment can do this on site.

 

ALREADY HAVE A VSS WITH ANOTHER ORGANIZATION/ BUSINESS:

VSS WITHIN THE LAST 2 SEASONS - NEED DECLARATION: If you have confirmed with Secretary and we have a copy of the VSS that you have submitted last season or within the last 2 seasons, you are required to complete our Criminal Offence Declaration Form.

STEPS REQUIRED:
1. Printout and complete the Criminal Offence Declaration Form (link)

2. Submit to Secretary
